TEAM
Transforming Episode
Accountability Model
Centers for Medicare & Medicaid Services (CMS) is launching the TEAM model on January 1st, 2026.
TEAM raises the stakes – and opportunity by placing all post discharge financial accountability on hospitals. Your Health helps hospitals win under TEAM by reducing avoidable utilization, improving patient outcomes, and ensuring long-term stability.
What is TEAM?
TEAM is CMS’s newest mandatory bundled payment model focused on surgical episodes.
It includes procedures like joint replacements, spinal surgeries, cardiac valve interventions, and select abdominal surgeries. The model holds hospitals accountable for total costs and quality during the 30 days after discharge. It eliminates conveners, incorporates equity adjustments, and raises the bar for documentation and real-time care coordination.
How Your Health Supports TEAM
Your Health is a proven partner for value-based care success. We help hospitals engage high-risk patients earlier, manage costs more effectively across the full episode of care…

From BPCI-A to TEAM: What Hospitals Need to Know
While CMS has experimented with value-based incentives for hospitals before – notably Bundled Payments for Care Improvement Advanced (BPCI-A) – TEAM marks a new chapter. With mandatory participation, regional benchmarks, and compressed timelines, TEAM presents both risks and opportunities for hospitals.
How TEAMS Will Impact Staff
The Transforming Episode Accountability Model (TEAM) will put hospitals at the center of patient risk because participation is mandatory and financial penalties are tied to post-discharge outcomes. Hospitals and their partners will be under more pressure than ever to deliver high-quality, cost-effective care — even after patients leave the building.
What Hospitals Should Look for in Post-Discharge Care Partners Under the CMS TEAM Initiative
CMS launches the Transforming Episode Accountability Model (TEAM)—and hospitals will be at the center of patient risk like never before. With mandatory participation, regional benchmarks, and a tight 30-day post-discharge window, success will hinge on choosing the right post-discharge care partners.
Prehabilitation: A Strategic Lever for TEAM Success
As healthcare leaders, we know elective surgeries are never “routine.” Outcomes, efficiency, and costs depend heavily on how well patients are prepared before they ever reach the operating room. That is the essence of prehabilitation: a proactive, evidence-based process of assessing and optimizing a patient’s health status before surgery to reduce avoidable risk and improve recovery.
Accountability Beyond the Hospital: Turning Communication and Data Into a Strategic Advantage
Value-based care is no longer just a talking point—it directly affects your bottom line. CMS’s TEAM and HRRP programs now tie reimbursement and penalties to post-discharge outcomes. For health systems operating on thin margins, even a small percentage in penalties can mean millions in lost revenue.
Let’s Succeed Together Under TEAM
The TEAM model shifts financial risk—and reward—to hospitals. Your Health helps your organization manage both. With smarter risk adjustment and cost-effective transitional care, we reduce readmissions and improve outcomes—without draining resources. Let’s partner now to protect your margins in 2026 and beyond.
