Becoming a patient begins with scheduling your first consultation, either in person, virtually, or over the phone. During this visit, we’ll review your medical history, discuss your goals, and create a personalized care plan that fits your needs. Our team handles coordination with your primary care provider or specialists, ensuring your care is consistent and uninterrupted.

From there, you’ll have access to our full range of services, including house calls, specialty care, remote monitoring, and mobile lab testing—all designed to make healthcare easier, more convenient, and centered around you.

What If I already have a Primary Care Provider?

We collaborate and communicate with your primary care provider, private care company, and Home Health Agency seamlessly. Your Health House Calls offers home care without conflicts, allowing you to continue seeing your primary care provider. For chronic care management patients, we seamlessly transition into becoming your primary care provider for comprehensive care continuity.

What If I don’t have a Primary Care Provider?

In such cases, we can step in and fulfill the role of your primary care physician, providing comprehensive medical care and addressing your healthcare needs.

Billing

We bill you the same way your doctor’s office does. If you have a co-pay currently, then you will have that same co-pay with House Calls. (We bill your insurance)

No contracts, agreements, or hidden fees. Any time our services are no longer needed, you can cancel at any time!

Insurance

We take Medicare Part B, SC Medicaid, and Private Insurance.

We do not take VA or Select Health, but we do take Tri-Care as secondary insurance.

    We are committed to delivering high-quality, patient-centric healthcare and wellness services, Your Health Organization offers a wide range of primary care options, including in-home visits, telehealth, and office visits, all designed to meet the diverse healthcare and wellness needs of our community.

  • We provides every service you would expect from a traditional Doctor’s office visit directly to the patient in their home. Patients can choose to see a medical professional in person via Telehealth, or at one of Your Health physician’s office. We provide services that include personalized care plans, medication management, ordering tests/labs, specialist referrals, follow-up appointments, wellness programs, remote patient monitoring, and much more. 

  • No, we are a Primary Care Physicians group. Still, we work closely with Home Health across SC/GA by collaborating with them and their patients to provide the best possible care.

  • We bill the same way as a Doctor’s Office, through insurance. No hidden fees or travel charges are added to your bill. For example, if you have a $20 co-pay at your physician’s office, you would have a $20 co-pay for us.

  • We have an online portal that you may utilize to see and pay for your bill, you can mail a check once you receive your invoice via email or mail, or you can call one of our representatives in the billing department.

  • Your Health promotes a proactive approach to healthcare. Through this healthcare model, we increase the quality outcomes and care a patient receives while decreasing the risk of unnecessary hospitalizations. We see our patients based on their needs a few times per month, either in person or via Telehealth, to ensure our patients are cared for appropriately.

  • We aim to have those orders taken care of for you. If there are orders that need to be handled, you can reach out to the account manager overseeing your building or the RN/MA for your facility.

  • Absolutely, particularly within Independent and Assisted Living Communities, we strongly recommend selecting Your Health House Calls as your Primary Care Physician.

    This partnership facilitates proactive care, fostering collaboration with the on-campus staff and medical team. Such coordination ensures swift communication, timely addressing of needs, and a proactive approach to prevent hospital visits. For residents, this means the convenience of receiving care within their familiar environment, eliminating the inconvenience of long waits in a doctor's office.

    Additionally, avoiding waiting rooms reduces the risk of exposure to airborne illnesses, prioritizing your health and well-being.

  • A Care Team comprises a Physician, Nurse Practitioner, Pharmacist, Nurse, Social Worker, and Medical Assistant who oversees and directs the care of your loved one. This care team works together to provide more comprehensive care for yourself or your loved one.

  • You can still see your Specialist and one of our specialty services. We currently offer Behavioral Health and Psychiatric services, Cardiac services, Diabetic Management, Palliative Care, and Clinical Pharmacy Consults.

  • Yes, Your Health works in collaboration with Hospices across both states to provide attending services for you or your loved one.

  • SC/GA House Calls works with multiple companies to provide point-of-service labs, which means the lab technician comes to you. This helps reduce unnecessary wait times and delays in getting lab work. We also work with a mobile imaging company that provides x-rays and other imaging services to our patients in their homes.
