Pharmacists

At Your Health, patients have access to a pharmacist who is a part of the care team. Your Health understands that medication problems are one of the main reasons people end up in the ER or the hospital.

Pharmacists conduct Telehealth or in-person visits in our Physician's offices as required to review medications with patients so that they understand their medication altogether. The pharmacist collaborates with the provider to recommend or assist in obtaining the appropriate drugs or discontinuing unnecessary medications.

Your pharmacist is available to answer any questions, as they understand that medication can be confusing. Your Health Pharmacists provide extra support to help patients feel better about their Health and wellness.
