In-Home Medical Care
Your Health House Calls nurse practitioners (NPs) provide in home medical services following a hospital stay or skilled nursing facility (SNF) stay.
Our NPs will assess the patient's needs, order medications and labs as medically necessary, and may send a summary of the visit to the patient’s primary care providers.
Our protocols for patient visits are based on risk adjustment factor scores assigned to patients to determine the number of in-home visits that aim to prevent unnecessary hospital visits and proactive health. In-Home medical appointments offer most of the same services associated with a traditional office visit, including mobile labs and mobile x-ray. Your Health House Call patients have 24-7 access to medical professionals, access to a pharmacist for medication reconciliations, and psychiatric specialists' availability.

Any service provided in a regular Doctor's office visit can be administered in a home visit. Your Health House Calls partners with mobile lab and mobile x-ray services when required.

Your Health House Calls does not charge trip fees to see patients in their homes. Insurance for the provider's service is billed the same as an office visit, with the patient responsible for their co-pay.

Yes, our partnership with Mako Labs allows bloodwork to be completed in the field with a fast turnaround of results sent to the provider.

A telehealth visit is a way for patients to receive non-emergency medical care at home and to avoid the inconvenience and discomfort of a trip to a doctor’s office, urgent care, or a hospital. The patient will also avoid spending time in a waiting room where there is the danger of exposure to the virus, the flu, or other infection.

Care might include: diagnosis, medical advice, testing, medication adjustments, treatment options defined, advanced care conversations, and additional services like home health or hospice ordered.

Any type of emergency care would not be available. A telehealth visit would not be appropriate to address bleeding, acute chest pain, shortness of breath—anything requiring an emergency room visit.

Some telehealth companies require a patient to be at least 18 years old. If an adult patient is unable to communicate, it is possible to have a telehealth visit with a family member or caregiver present to speak and act on their behalf.

As a courtesy to our patients, Your Health House Calls will submit claims to your primary and secondary insurance providers on your behalf. However, the primary responsibility for the account is yours. Please remember to bring your most recent insurance card to your next appointment. Companies not listed will require you to pay for your visit prior to seeing our medical provider. To maximize your health insurance benefits, please contact your insurance company’s customer service department for policy and benefits verification. Also, to ensure that our provider is listed as your PCP. The customer service phone number should be located on the back of your health insurance card.
