Specialists provide care through scheduled telehealth visits that are facilitated in the home by members of your in-home care team. During these visits, the in-home team members support the specialist by collecting real-time vital signs, reviewing medications, assessing function, and sharing insights about the patient’s environment and daily challenges.

Because specialty care is fully integrated into the care team, recommendations can be acted on immediately. Specialists collaborate directly with primary care, pharmacy, behavioral health, therapy services, and palliative care to ensure each plan is practical and coordinated.
