How Remote Patient Monitoring Helped Save a Life

At Your Health, our mission is simple: keep patients safe, supported, and out of the ER whenever possible. Recently, our Remote Patient Monitoring (RPM) program did just that—helping detect a life-threatening cardiac event and ensuring a patient received the urgent care they needed.

It started like any other day for our RPM Coordinators, Latoya Spruill and Melissa Hardman. Latoya noticed an unusual alert come through. The patient’s numbers were far outside their normal range. Instead of dismissing it, she dug deeper, checked the patient’s records, and called the patient’s son, who was also the caregiver.

The son explained his mother hadn’t been feeling well, but he didn’t recognize the seriousness of her symptoms. He re-checked her vitals, and again, the numbers were concerning. Latoya quickly escalated the alert to the patient’s care team. Soon after, it was confirmed: the patient was in cardiac distress. Thanks to the quick action of the RPM team, she was taken to the ER immediately—likely preventing a tragedy.

That human connection is at the heart of RPM. Every blood pressure reading, every weight check, every small data point builds a fuller picture of a patient’s health. Small changes can point to big problems—and our coordinators are trained to spot them.

As Latoya explained, “It’s cool to know I’m still making a difference. Even if it’s over the phone, patients often say, ‘Thank you for calling me.’ That means a lot.”
