Four Specialty Medical Services for Enhancing Resident Wellness in Assisted Living

Multiple studies have shown a correlation between regular healthcare access and the reduction in hospitalizations. In addition to its primary care service, South Carolina & Georgia House Calls (SC & GA HS) has integrated over ten specialty services into its assisted living (ALF) offering. These services provide ALF residents with a complete care solution onsite. Below are four specialty services and how they can enhance resident care.

 

Cognitive Behavioral Therapy Specialty:

Cognitive-behavioral therapy assists residents with chronic illnesses and manages symptoms to improve quality of life through positive psychosocial outcomes. The licensed social workers provide patient counseling by working closely with the psych specialists. Conditions followed include depression, anxiety, psychiatric diagnosis, and complicated grief.

 

Diabetic Specialty:

SC & GA HC uses a dedicated specialist team concentrating on all diabetic patients under their care. This team works closely with pharmacists who provide specialized medication consults. Residents assigned to this program would be identified by elevated glycated hemoglobin (A1Cs) that are not decreasing, increasing, or for those requiring constant medication monitoring. 

 

Pharmacist Consultation Specialty:

Pharmacists working with the primary care provider help mitigate hospitalizations caused by medication non-adherence. Consults include medication reconciliation, polypharmacy evaluation, de-prescribing, and resident education on current medications being taken. The consultations often lead to savings when unnecessary drugs are eliminated.

 

Cardiac Specialty:

The cardiac team within SC & GA HC specializes in helping residents manage congestive heart failure, hypertension, and other related heart conditions and diseases. Remote Patient Monitoring is used extensively as a critical tool in monitoring daily vitals for these residents.
