With over 1,600 providers and a commitment to value-based, patient-centered care, our specialties support patients wherever they are—at home, in assisted living, or via telehealth.

1. Intake Specialty

Every new patient begins their care journey with the Intake Specialty team, which conducts a telehealth visit within 24–72 hours of referral. These visits focus on medication reconciliation, a review of the patient’s health history, and an assessment of their current condition—equipping in-home providers with the insights they need for a highly productive first visit.

2. Cognitive Behavioral Therapy Specialty

Our team of Master’s-level social workers, based in the Center for Telehealth in Columbia, provides virtual counseling and behavioral health support. Working closely with psychiatric providers, this team assists patients managing depression, anxiety, psychiatric conditions, and complicated grief—delivering compassionate care tailored to each individual.

3. Pharmacist Consultation Specialty

Medication-related issues are a leading cause of hospital readmissions. Our pharmacist team collaborates with primary care providers to improve safety and adherence. Services include medication reconciliation, polypharmacy evaluation, deprescribing, and patient education—helping reduce avoidable hospitalizations.

4. Palliative Care Specialty

Our Palliative Care team (also known as Supportive Care) provides specialized medical care for those with chronic or serious illness. This care focuses on relieving symptoms, pain, and stress while improving quality of life. It can be delivered alongside curative treatment and at any stage of illness.

5. Hospice Attending Specialty

Our providers work directly with partnering hospice organizations to ensure high-quality care for patients receiving hospice services. This includes face-to-face documentation and close collaboration with in-home providers to support both patients and hospice teams.

6. Diabetes Specialty

With diabetes impacting more than 34 million Americans, our Diabetes Specialty team offers proactive, ongoing care. Focus areas include hemoglobin A1C screening, management of complex conditions (such as wounds, kidney disease, stroke), and dietary support provided by registered dietitians.

7. Dietitian Specialty

Our registered dietitians support patients managing conditions like diabetes, hypertension, high cholesterol, and hypokalemia. They provide Medical Nutrition Therapy (MNT) and collaborate with providers to set clear, measurable dietary goals—also assisting patients with feeding tube nutrition plans.

8. Home Health Specialty

Led by physicians, our Home Health team establishes strong provider-patient relationships and supports ongoing care for patients enrolled in home health services. This includes monthly care plan oversight, recertifications, and dischargeswhen appropriate.

9. Psychiatric Specialty

Your Health offers psychiatric care as an integral part of our telehealth and in-home services. Providers work closely with primary care teams to treat mental health conditions including depression, anxiety, grief, and other recognized psychiatric disorders.

10. Cardiac Specialty

Our Cardiac Specialty team manages and monitors patients with heart-related conditions using tools like Remote Patient Monitoring (RPM). They focus on issues such as hypertension, heart failure, arrhythmias, chest pain, and high cholesterol—helping patients stay safe and stable at home.
