Visiting Nurse

At Your Health, patients can receive the added support of a Visiting Nurse as part of their care team. The nurse may visit in-home to assist patients with questions, concerns, or issues. They also serve as the eyes and ears for the patient's Nurse Practitioner or Physician Assistant between visits. The ultimate goal is to keep patients home and avoid costly emergency room visits or hospitalizations.
