Community Health Worker

As a Patient, you will have a community health worker responsible for ensuring you receive your medications and any necessary medical equipment from the Your Health Pharmacy. They also offer assistance with setting up or troubleshooting Remote Patient Monitoring equipment if needed. Furthermore, the Community Health Workers are always available to listen to you and communicate concerns to your care team so you have the health success you have been looking for.
