As a part of our team, you will:

  • The Regional VP of Case Management at SC/Ga House Calls is responsible for ensuring that patients receive the right visit at the right time by the right provider, that staff are productive, that appropriate tools are used to promote proactive healthcare, and that care teams are appropriately staffed. This includes monitoring and promoting the appropriate use of tools such as chronic care management, transitional care management, advanced care planning, screening for cognitive behavior, labs, x-rays, and other screening tools. The Director of Case Management will supervise the Director of Clinical Services, who in turn supervises the care teams consisting of case managers, clinical coordinators, and providers.

Must Haves:

  • Bachelor's degree in healthcare administration, social work, nursing or related field; Master's degree preferred.

  • Minimum of 7-10 years of experience in case management or related field, with at least 3 years of leadership experience.

Job Type: Full-time

Pay: $80,000.00 - $100,000.00 per year
